This survey is intended to collect feedback regarding the Day Services you or the person you support receives. Day Services include Individual Employment, Community Engagement, Community Inclusion and Respite.This survey is interested in understanding more about your experience receiving community inclusion, community engagement, or supported employment services. We want to hear from everyone who has direct experience with these programs, and we want this feedback to be as representative as possible of people who are directly impacted by these programs. If you know someone who you think should complete this form, please forward this link. The form may be completed by the person receiving services or their representative on their behalf.
